Custom Error Pages.

How do some sites get their own custom "File not Found" pages? Is it simply a page with certain name?

There's an error template, that you dont get in lycos to my knowledge.

With Apache, it's located (if at all) in the file ".htaccess" (if existant).

Don't know the rest.

It's simply a matter of the server having a spec in one of its config files that tells it to redirect to a certain file when it's about to return an error code to the browser. If there is no file specified, it simple returns the error code to the browser, causing the browser-specific error message (e.g. HTTP 404 Not found) to be displayed.
Most proffesional servers allow this, most free ones don't. I don't think that Lycos supports it.
