About outputting a square wave in QB:
If you are in "vanilla" DOS or Win9x and have a SoundBlaster 16 compatible card you can control the FM sinthesyser (spell??) chip, by using out commands
If you are in Win XP/2000/Nt, you can get VDMSound, a dos box that provides a SB16-style interface to any card you have. This way you can use that same out commands.
Then you will need to learn how to program the FM synthesiser. Here is an example from the ABC packets
